Subject: Attention: Kaiser Permanente Patients Who Are Blind or Have Low Vision

Disability Rights Advocates and the law firm Goldstein, Demchak, Baller, Borgen & Dardarian are investigating complaints regarding barriers that people who are blind or low vision have experienced in Kaiser's programs, services and facilities.

If you have been unable to access Kaiser's website or to get important information in alternative formats, such as medical forms, prescriptions and educational materials, we would like to talk to you. In addition, if you have faced architectural barriers or staff insensitivity to vision disability issues, please contact us.

To share your stories, email kaiser at gdblegal.com, or call 866-723-1494. All communications will be kept confidential.

Thanks for any information you can share.

Sincerely,

Laurence Paradis

President and Co-Director of Litigation
