in terminal mode on my mac i can run
SQLite3
it is light weight, but recommended for smaller applications.
run it at it's dot prompt or use a language like python to communicate with it.
it may already be installed on your mac or you can install the command line tools to get it.
at it's dot prompt type .help and .quit to end.
do you know SQL?

We are looking at using a database that can receive inputs from a web form and put the information into the correct database based on selections made. Are there any recommendations people know of that would work?
