[Nfbf-l] seeking advice on possible discrimination

     I have been attempting to complete a form in which I am filing a complaint against a veterinarian here in Florida. I have tried to fill out the paperwork on the agency website, but it is inaccessible with my jaws program.  I contacted the agency for assistance, but they said they do not have the time to assist me with the entire form, and subsequently said I could not have the form completed over the phone because they could not prove it was written by me.  When I pointed out that their form was not accessible to the visually impaired, they said they were sorry and told me to find a sighted person to help me fill it out.  I told them I did not have anyone to help me right now and I felt this was discriminatory.  They still said they could not assist me. 

     Does anyone have any knowledge or advice how I can proceed with attempting to get the agency to comply with the ADA in offering me a reasonable accommodation for filing my complaint?  The office is not local and to go into their office  I would have to travel to Tallahassee from Gainesville, which is certainly cost prohibitive.  Any help in trying to resolve this matter would be appreciated.  The complaint against the veterinarian is very serious and I certainly would like to pursue it.
