I have copied various community members who might be able to assist with providing Self Reliance with names of individuals who are 65 y/o and above in the following zip codes who may be interested in receiving the COVID vaccinations.  The purpose is to pre-register the individuals to receive the vaccination.    I understand individuals would need to provide you with the First Name, Last Name, Date of Birth, Phone, Mobile, Email, Address, City and Zip. As you have heard, the Federal government is establishing a vaccination site on March 3, 2021 at 755 E Waters St, Tampa.  This site is specifically designed for vulnerable populations in certain zip codes where there is a Social Vulnerability Index (SVI) of .5 or greater and a high population of 65 y/o + or greater.

This morning we were given the opportunity to "pre-register" some of our citizens prior to the registration being opened up.  We are reaching out to you in hopes that you can use your contacts to quickly identify and provide information for citizens in these zip codes who may want the vaccine.

For the pre-registration we only need very basic information about each individual.  Please identify individual and enter their information on the attached spreadsheet and return to me no later than 4:00 p.m. Thursday (Feb 24, 2021).  I will consolidate the list and forward to Emergency Management.

I realize this is short notice and a quick turnaround but we wanted to take advantage of the pre-registration option.
