[NFBK] Registration Form for the NFB Kentucky State Convention

Good Morning, Everyone:

 

We are finding registration forms for the NFB Kentucky Convention that do
not have complete information. All fields are required for your form to be
accepted to participate in the NFB Kentucky state convention taking place on
10/2 and 10/3. In addition to your name, we require address, city, state,
zip, phone and email fields to be completed. If you need assistance, please
let us know so that we can ensure a complete registration form. You will not
be allowed to participate in this convention if we do not have the
information required on this form.
