the first year that The National Federation of the Blind of Greater Louisville

National Federation of the Blind of Greater Louisville’s scholarship has been
named the Mittie Lake Memorial Scholarship in honor of Ms. Mittie Lake, a
charter member of the National Federation of the Blind of Greater
Louisville.<SPAN style="mso-spacerun: yes">

award scholarships in amounts up to $1,000.<SPAN
style="mso-spacerun: yes"> </SPAN>The Scholarship committee reserves the
right to award either a $1,000 scholarship to one (1) individual or two (2) $500
scholarships depending upon the needs of the applicants.<SPAN

Scholarship</SPAN></SPAN></P>

style="mso-bookmark: ole_link1"><SPAN style="mso-bookmark: ole_link2">The Mittie
Lake Memorial Scholarship is named in honor of MIttie Lake, a long-time
Federationist and charter member of the National Federation of the Blind of
Greater Louisville. Mittie was a student at the Kentucky School for the
Blind.<SPAN style="mso-spacerun: yes"> </SPAN>During her middle
years, Mittie lost all of her sight.<SPAN style="mso-spacerun: yes">
</SPAN></SPAN></SPAN></P>
<P class=Body style="MARGIN: 0in 0in 10pt; LINE-HEIGHT: 14pt"><SPAN
style="mso-bookmark: ole_link1"><SPAN style="mso-bookmark: ole_link2">Mittie had
an enthusiasm for life and all of its many offerings.<SPAN
style="mso-spacerun: yes"> </SPAN>The fact that Mittie became totally
blind did nothing to stop her from living the life she wanted.<SPAN
style="mso-spacerun: yes"> </SPAN>Mittie was married and raised a
family.<SPAN style="mso-spacerun: yes"> </SPAN>She worked tirelessly in
her church nursery for more than thirty years, providing nurturing care for
hundreds of babies and young children.<SPAN style="mso-spacerun: yes">
</SPAN>MIttie sang with</SPAN></SPAN><SPAN style="mso-bookmark: ole_link1"><SPAN
style="mso-bookmark: ole_link2"><SPAN lang=FR style="mso-ansi-language: fr">
numerus choir, quartets and ensembles.<SPAN style="mso-spacerun: yes">
</SPAN></SPAN>She was a strong advocate for Braille and its use in all aspects
of daily life.<SPAN style="mso-spacerun: yes"> </SPAN>Mittie chose to use
a guide dog and treated her guides not only as the tools she needed to live her
life independently, but as valued friends and companions who were never without
love, affection, and a treat or two.<SPAN style="mso-spacerun: yes">
</SPAN></SPAN></SPAN></P>
<P class=Body style="MARGIN: 0in 0in 10pt; LINE-HEIGHT: 14pt"><SPAN
style="mso-bookmark: ole_link1"><SPAN style="mso-bookmark: ole_link2">In honor
of her life and story, preference will be given to applicants who are guide dog
users and who utilize Braille as part of their daily lives.<SPAN

individuals who are attending or will be attending post-secondary institutions
in Jefferson County during the 2016-2017 school year.<SPAN

Jefferson County or be students at the McDowell Center who have paid their dues
to the Greater Louisville Chapter prior to applying for this scholarship.<SPAN

visually impaired and/or eligible to receive services from the Kentucky Office
for the Blind. Proof of legal blindness or a statement of eligibility from the
Kentucky Office for the Blind must be provided as

attend an accredited college, university or on-line program that is based in
Jefferson County.</SPAN></SPAN></P>

County and/or attend a post-secondary school in Jefferson County. Students who
participate in on-line programs based in Kentucky, but do not have any physical
ties to Kentucky, i.e. are not physically in Kentucky, are not eligible to apply
for these scholarships.</SPAN></SPAN></P>

style="mso-bookmark: ole_link2">Scholarship finalists must participate in at
least one activity of the National Federation of the Blind of Greater Louisville
prior to the awarding of the scholarship.<SPAN style="mso-spacerun: yes">
</SPAN>This activity may include but is not limited to a monthly meeting,
community outreach activity or social activity sponsored by the Greater
Louisville chapter of the Federation.<SPAN style="mso-spacerun: yes">

style="mso-bookmark: ole_link2">Scholarship finalists must attend and
participate in all activities at the annual state convention of the National
Federation of the Blind of Kentucky. The NFB of Kentucky state convention takes
place annually during a weekend in late September or early October. <SPAN
style="mso-spacerun: yes"> </SPAN>Convention expenses are covered as a part
of this scholarship program. Scholarship winners will be chosen during the
annual State Convention. Information in regards to convention date and location
is forthcoming.</SPAN></SPAN></P>

Scholarship finalists who are invited to attend the annual NFB of Kentucky State
Convention are not guaranteed a scholarship. The scholarship committee will make
final decisions regarding scholarship awards during the convention. However,
even if finalists are not chosen to receive an academic scholarship, Convention
expenses, including Convention registration fees, hotel accommodations, and
three meals on Saturday of the convention will be awarded. Scholarship finalists
are responsible for other expenses incurred during the
