Hi Michelle,

We created an online survey for all of our members to find out what are some of the barriers they face, what topics they would like to see at chapter meetings, and how, as a chapter could we best meet the needs of our members.

The survey was created through Survey Monkey. Latonya Phipps was the one who made sure the survey would be accessible. You can contact Latonya at latonya.phipps at cms.hhs.gov.

I was reading from an old email from 75 Days of Action and read that the TLC Chapter had an on-line survey at one time. Can I get a contact for whomever is monitoring this or did in the past?
