Dear Sir,
I’m writing you today to request the name, address, phone, and e-mail of LARA’s “ADA coordinator” and for an accessible copy of the required departmental grievance procedure. (Please see Americans with Disabilities Act of 1990, Title II, regulatory citations after my signature line.)
Moreover, as a person who is blind I am requesting prompt and timely access to this and other information in accessible format pursuant to obligations under Title II, effective communications, subpart e. To wit all correspondences in this matter may simply be done through conventional e-mail as I use screen reading technology. You may simply remit this information or any questions related to it by responding to this message and or you may simply remit requested information as a Word or other screen reader accessible document a file attachment to my e-mail address listed above.
I understand that LARA is a recent organizational restructuring, but these are civil rights laws and I’ve had several complaints as you know and no one qualified to contact let alone remedy them with which is why these are federal, long standing obligations. By the way there are similar requirements under Section 504 of the Rehabilitation Act.

35.107 Designation of responsible employee and adoption of grievance procedures

(a) Designation of responsible employee. A public entity that employs 50 or more persons shall designate at least one employee to coordinate its efforts
to comply with and carry out its responsibilities under this part, including any investigation of any complaint communicated to it alleging its noncompliance
with this part or alleging any actions that would be prohibited by this part. The public entity shall make available to all interested individuals the
name, office address, and telephone number of the employee or employees designated pursuant to this paragraph.
(b) Complaint procedure. A public entity that employs 50 or more persons shall adopt and publish grievance procedures providing for prompt and equitable
resolution of complaints alleging any action that would be prohibited by this part.
