This is all I requested a long time ago. By the way I can't scan all that is requested and again I wish to know who handles Americans with Disabilities Act requirements for your bank?
Regardless this is at least a start.
Sincerely,

Dear Joe,
I just wanted to follow up with you per our conversation on the phone.
The bank is required to follow certain government reporting when opening accounts for individuals, business' or organizations. This reporting is called the Client Information Program. Simply stated when opening accounts we need to accept a valid copy of a drivers license, state id or passport and tax id number to complete the process of account opening.
For your conveinecne you can scan and email myself a copy of your state id along with your social security number to the email address listed below or stop in at any local FirstMerit Bank Branch and speak to a representative who can assist you in getting the information to me. Simply give her my name, Valerie Bader and a contact phone number of 810-631-2135.
If there are any other further questions you have for me please do not hesitate to contact me at 810-631-2135 or through email.
Enjoy your evening.
