Brian,

Are you referring to the speech capabilities in the Apple iPhone, or just usability issues if you are visually impaired? 

As far as speech, the iPhone has a ways to go. 

For low vision, its pretty darn nice. The new retina display on the iPhone 4 is very nice to view.
