Hi, folks. For those receiving letters about the Blind Pension program,
please look at this note. It contains an important correction.

I learned from MO HealthNet that the fax number to send the completed Part D
form is not the same on the letter as the form itself. The correct fax
number is 573-522-8514. The form includes the prefix 751 rather than 522.   

Anything you can do to help correct this is appreciated. Again, the correct
fax number is 573-522-8514.
