talk to an Eligibility Specialist call the main information line at 
855-363-4636.
There may not be a problem as forms are sent out 11 months after the office 
last completed a review. So, if you sent a review in to the office  in may 
and they didn't complete it until July the next one would be sent out in 
June.  It is actually more involved than that but that is the basic 
possibility.
By the way forms being mailed in June will be mailed this week, probably 
Monday or Tuesday.
