Well, after getting kicked out of the on line questionnaire for five times,
and once when I only had one more question to answer, I gave up and called
the number 844-330:2020, went through a couple of automated prompts, then
pressed zero for a representative, who answered almost immediately and took
my information.  It is so nice to have it over and done with.  Joy
