Go to www.nfb.org/rideshare and click on the online questionnaire link. It
will bring up a ride share experience questionnaire that you will fill out
and submit each time you take either Uber or Lyft, whether your ride
experience is positive or negative. It will ask such things as your name,
email address, which ride sharing company you use, whether you have a guide
dog, and how your experience was, whether the driver treated you
respectfully, etc.

I filed out the ride and wonder if there is a way I can find it easier to
fill out again?
