[NFBofSC] Update RE: Talking Medical Device Survey Form

Hi List,
I had made some changes to the survey form that made it easier to read with JAWS. These updates were submitted several months ago, but in light of recent events, I am sure it was an easy oversight. Nothing of great substance was changed, but, the sections for income and conditions to track were separated to keep the sections separate for a JAWS user. JAWS will read straight across, and though columns were evident to print readers, things were not as straightforward with JAWS.
Until this is able to be put on the website, you can save the document and fill it in on a computer fairly easily. Just print it out to sign and date before submitting.
