Kaiser Health News is conducting a survey on the experiences of blind people
with inaccessible medical bills, statements, and documents. Please note that
this is not an academic survey but one for a news organization; consequently
your responses will go to reporters. However, we are confident that this
news organization has integrity and will not share personal information
without the permission of the individuals providing it. The National
Federation of the Blind provided feedback on the survey and believes that a
news story about this issue would help to increase public awareness of the
