Some of you are aware that I was denied services because of issues centered
around Department of Rehabilitations ridiculous requirement for a wet
signature on the 2660 Form. I think the gist of this form is to release
confidential information to authorized vendors such as the SF LightHouse for
the Blind/Visually Impaired. In August, I approached Disability Rights
California. At the last Blind Advisory Committee meeting I addressed the
Advisory Board and, I believe, Joe Xavier. Many of you heard President Tim
Elder address this issue with the Director of Rehabilitation during the NFB
State Convention. Today, I received the following letter from my DOR
counselor. I have removed their name. I am sharing the letter in case you or
someone else is faced with denial of services as a result of a wet
(pen-to-paper) signature requirement.

Good news, DOR is accommodating our consumers with alternative to requiring
a "wet signature' on the DR 260 Consent to Release and Obtain Information
form. 

 

You are invited to utilize email correspondence to electronically sign the
attached document. By signing the attached document electronically, you
agree to the following:

. You confirm that you are the person identified on the attached document
and sign on your own behalf.

. The document provided in this attached email is the document that you are
signing with an electronic signature.

. No changes have been made to the attached document that you are signing.

 

If you agree with the attached document and the above statements, please
send an email to me stating "I agree" with the document attached. Your email
with the document attached will serve as your electronic signature. The
document provided in this attached email is for your personal record. The
Department of Rehabilitation will also keep a copy of the document and your
accompanying email correspondence electronic signature.

 

If you do not agree with these conditions, or do not want to sign the
attached document electronically, you may decline. If you would like to
propose changes to the document, please contact me or your counselor to
discuss those changes.

 

Please know that you may refuse to electronically sign other documents to
carry out future business with the Department of Rehabilitation.
