[NFBWV-Talk] Registration form update

Thanks to Caron McDonald, I have made the convention registration form
easier to complete (especially for those that might have had some
difficulties completing it earlier).

I have changed some of the fields from drop-down list fields to radio box
fields.

With that said, you can resume completing the convention registration form
and the room reservation form for the upcoming state convention.
