find where JAWS will say the word edit.  if you don't hear a popping 
sound, which indicates forms mode has been activated, press the space 
bar to enable forms mode.

once you've typed in your first name, press the tab key to type in your 
last name, and so on.  when you need to fill in your phone number, just 
type the numbers, don't worry about typing the parenthese and dash. 
when you come to a combo box, you should be able to use the down or up 
arrow keys to your choice, and press the tab key to move on to the next 
bit of information you are asked.
you should eventually land on a submit button where you'll press the 
space bar to send the completed form.
