[NJTechDiv] renewing your affiliate form link

I’m very confused about membership renewal.  I paid my dues online before convention, and I thought that renewed my membership, but I didn’t see anywhere to put a chapter, only my committees and groups.  

So do I have to fill out the form again, and put 0 in the dues part, because I already paid?  And do I have to apply to a chapter for membership?  I’ve been in the At Large chapter before.
