We were discussing last week about entry into Hawaii from the mainland for US Residents with a guide dog.
This was sent to me, back in July of 2011, so it is the latest information.
Peter if you can add this to the travel and tourism website,
Margo, I believe that NAGDU has this information, but I am sending it to you just the same to post to NAGDU as well.

> Hi,

> Please use the following link for information: http://hawaii.gov/hdoa/ai/aqs/guide-service-dogs-entering-hawai-i on requirements and qualification requirements for guide and service dogs that are not for emotional support, comfort, or therapy.
>
> Please fax the information to 808-483-7161 begin_of_the_skype_highlighting 808-483-7161 end_of_the_skype_highlighting and list contact phone numbers and your e-mail address (e-mail is answered in a different building at a location several miles away) so that the dispensary staff can contact you. Conversely please call them if you need additional information. You can reach the dispensary by calling 808-483-7154 begin_of_the_skype_highlighting 808-483-7154 end_of_the_skype_highlighting. They are often very busy so you may have to leave a voicemail message. It is critical that we pre-qualify you and your service dog under our service dog program prior to entry.
>
> If this e-mail message bears a statement that we do not accept faxes please disregard that. We do accept faxed information for qualified Guide and Service dogs.
>
> Basically what is required is a letter from your physician stating the need for the animal and what it does for you to mitigate or service your disability. It needs to be on the Doctor's letterhead and not on a prescription pad. Verify that you are qualified to enter under the "modified" program for Guide and Service dogs. Dogs entering under that program must bear a microchip (AVID or Home Again are preferable - if using another brand of microchip please check if it will be accepted with our system), have a passing OIE-FAVN blood test, and a current rabies vaccination. All of these must be completed before arrival. There is no waiting periods following the blood test or rabies vaccination for guide or service dogs qualified under our program.
>
> Once accepted under the modified program and the dog is prepared for entry fax the staff your travel itinerary to the number above, at the very least a few days before arrival, and stay in touch with the veterinary staff at the second number listed above. In your client's case they must make special arrangments with our staff as it is not normal for us to do direct releases on cruise ships and normal entry is via Honolulu.

> Do not send e-mail that is not related to quarantine issues. We will add your message to our spam folder if we get ads or non-quarantine related messages. Once we add you to our spam list we cannot reverse it.
