1.	Fill out the Accessible Witness Information Form (Attached)

1.	This form will ensure that any accommodations are met and your information is correct.

 

1.	Submit Your Testimony

Due to the committee being next week it is imperative that your testimony is submitted 24 hours prior to the hearing.
