[Ohio-talk] Order form for HOODIES!!!

Hi everyone, attached is the order form for the hoodies being sold by the Columbus chapter. The hoodies are as follows
pullover or zip up
royal blue or black
NFB on the front left (when wearing)
NFB logo large on the back
 
If you have more than one order per person please fill out multiple order forms. You can pay with one check, but please include all names in the memo of the check. Please send all payments with order forms to Stephanie Claytor. Her contact information is on the order form. Payments must be received before state convention.
