It's not quite as serious as you believe it is, most of the large builds 
are very well tested before they even make it to a standard user. Jaws 
works well enough, and there are always other screen readers, NVDA comes 
to mind which really work well. I've been using 10 even before public 
release, and I flag issues as I see them for any of the screen readers.
