I don’t know about the PDF forms except that you can print them out and fill them out and then scan the finished form back to the entity that wants it. That is usually what they expect us to do, and it is a pain.
About the AZ affiliate. Don Porterfield is the president, but I don’t have contact info for him. Bob Kresmer is the past president. He would still be a good contact. His home number is 520-733-5894, and his email is krezguy at cox.net.I <mailto:krezguy at cox.net.I> think he could and would put these parents in touch with the right people.
