Here is the form! This form isn't required however, if you want to come
testify in person you can submit your testimony and give me a heads up or
you can just simply show up and testify when the Chairman asks the audience
if anyone else is there to testify! These meetings are open to the public,
the testimony and witness forms are simply just a nice heads up!

Chair Lipps, Vice Chair Manning, Ranking Member west, and Members of the
OHIO House Health Committee Thank you for the opportunity to submit
testimony in support of HB214.

I lost my eyesight when I was 15 years old and now, I am 52. Until recently
it was not necessary for me to have medication but with back pains and other
serious medical issues it has become very important for me to know what meds
that have been subscribed and the side effects. I  take on average three
different medications each day. It seems that the drug companies are also
making most tablets in the same shape. When I first started taking the
medications you could tell the difference in the shapes but that is not so
easy now.   There  was even a time even  once when I got medications mixed
up and could feel the side effects even more.

 

I would like to tell you another story that affected me in a major way.
Three years ago, my uncle in Arkansas got very sick and had to take several
different medications other members of my family chipped in  to see what
they could do  in order to assist him. For several days he and I had to stay
alone and even though I go all over the united states telling blind people
how independent they can be and that they are first class citizens not being
able to administer his medication with the same confidence that I have in
all other areas of my life made me feel like a helpless child. I hope you
will do everything that you can to get this bill written in to law. Thanks.

not critical for you to testify in person if there are some technical
issues, you can still testify without sending us anything over prior

                I do not have a witness form or testimony on record for you
in regards to tomorrow's hearing. Should I have received info from you? The

Good morning,

        You will need to fill out a witness form to testify in person
tomorrow. If you would like committee members to have a copy of your
testimony the witness form will need to be filled out in advance along with
a file of your testimony. Additionally, you can come to the committee and
fill out a witness form in person, without sending us anything in advance!
Whatever is easiest! I have attached the blank witness form!
