Short Adult assessments

I am looking for assessments that can be used to determine
skill levels for adults in a short duration rehab program. I would like them to
establish an initial level of skill in an area, as a baseline, and upon
re-administration, give an achieved skill level at the end of the program.
Specifically, I would like them in DLS, O&M, and braille, but would welcome
them in other areas.

You can reply to me either here or off list at the address
and phone below. Thanks.
