When I worked as a consultant for a disability rights organization in Indianapolis Indiana, I was not required to submit proof of insurance, only prove that I was eligible to work in the United States, and agree that I would be responsible for my own transportation's, but would be compensated the state allowance for mileage. It was up to me to then work out the cost of a driver, and how I made it to and from appointments.
