[rehab] Questions for rehab instructors

Hi all!  I am serving on a committee to revamp the training program at our state rehab center.  We're currently looking at revising our report writing methods, and are hoping to include more checklists, rating scales or other assessment templates to evaluate student progress.  Rather than reinventing the wheel, we're reaching out to other center's for the blind to see what they've found helpful.  If you'd be willing to share some examples of what you're using, it would be much appreciated.  Feel free to reply on or off list; since attachments probably can't be posted to the list.
