[SportsandRec] Ax throwing waiver pre-sign

Hello Federation members,

Tonight, the Sports and Rec division is hosting an inclusive carnival! Join us in the surface parking lot and Ballroom D for a night of fun. We have 64-ounce games in Ballroom D, and outside, you'll find axe throwing, climbing, bungee running, a bounce house for the kids, and several demo stations to try various blind sports.

If you're interested in participating in axe throwing, please fill out the waiver below beforehand to save time at the event. A suggested donation of $10 is appreciated. Enjoy the night and join us!
