There is a header function, at least in the version of MSWord I use. 
“Header and Footer” is on the “View” menu. If you don’t have a menu, I 
don’t know what to tell you. But, there must be some way to access the 
functions that were on menus in previous versions. Isn’t there a ribbon 
or something?

Likewise, you should be able to format the number text to include the 
required info and, of course, to place it in the upper right. In my 
version of Word, you insert page numbers on the “Insert” menu. It is 
probably on the equivalent ribbon.
