[stylist] Seeking young adult novel

Okay, years ago I read a Young Adult fantasy novel but can't remember
the title or author. So far, I have yet to meet anyone who recognizes
the description, and I've tried some online searches with no luck.
Here's a description of the story, and if anyone recognizes it, I would
appreciate the title and/or author if you know it.

A teen girl from a fantasy world visits her friend who keeps watch at
the bottom of a wishing well. She is somehow transported to our world
through the well where she meets a teen boy who lives with his grandpa
and sister. The girl has no clue how to get back and must learn to live
in our world. As it turns out, the boy's grandpa is from the girl's
world, but he fell in love with a woman from our world, hence his reason
for being here. The girl and the boy fall in love, but by the end of the
book, she has returned to her world. I believe it ends with the boy
looking down into the wishing well and the girl looking up, both trying
to figure out how to get back to one another.

If anyone recognizes this, please let me know. This has been driving me
crazy, grin.
