Hi Amy, it would be worth checking that full keyboard access in Accessibility Settings is disabled. That setting causes no end of issues with VO.

Subject: [Trainer-Talk] question about interference with VoiceOver
Hi,
I have a new client whose family may have set up some customizations that are interfering with VoiceOver, as gestures are not interpreted properly. I have re-started the phone and know to turn off voice control, but wonder if there are other things I should look at. I usually set up phones for people, so this is a relatively new experience. This person has declining vision and needs /voiceover all the time.
Thanks.
Amy
