You most definitely want to use your travel certificates before they expire. 
Now to stay with American Express is an individual decision.  I have a 
frequent flyers credit card with Discover that works much like American 
express without the yearly annual fee.

My husband and I use the American Express charge card as much as we can to
increase our frequent flyer miles, and we pay it off at the end of every
month.  I have been debating about whether to continue with them, as they do
charge a $150 fee at the beginning of each new year, but we do have some
companion fare certificates that we wouldn't be able to use next year unless
we paid that $150.
