I sent you an e-mail off list, but  I will also answer your questions here.
Yes, Jamaica you cannot enter Jamaica the dog is from the U.S. and they only allow dog from the U.K.
The Cayman Islands, spelled C a y m a n, pronounced Kay-Man.
I know I just finished a travel specialist course on it, and was recently at a Cayman Island Travel event that I am currently writing up for my blog and to the list.
You are not totally correct on that matter, and they tourism board that was at that event are looking into all of it for me, especially since one of the tourism board member has a wife here in the us, since they were born here and work there, uses a service animal. She is not blind but uses a service animal.
It is spelled N A S S A U, then you should have found another cab, that is against the law, especially in the Bahamas.  I hope you had your USDA Health Certificate with you and the vaccinations as well. Both times that I was in Nassau, back in October 2011 and this past April 2012, my guide dog went with me and had no issues and no way did they ask $60.
I know many of  the taxi owners there in the Bahamas, especially the ones right outside when you walk into town.  So I will be on a call with them this afternoon to verify that.
There is also Puerto Rico, St. Thomas, St. Maarten, you can go to, Maxx my guide dog has been there as well.
