It's been a long time since I've done much air travel, and things change. So
this may not be accurate any longer, but I don't remember ever having
trouble with Southwest. They also used to have an option where you could pay
an extra $25 and get a seat with extra floor space. It was good guide dog
space, not tall person space. It took advantage of the curve of the plane. I
also never had trouble with US Air. 
I have also taken the train, Amtrak, long distances as an alternative to
flying. Rates on Amtrak are rather high in the northeast, but going longer
distances or in different directions was not as expensive. I've been several
times between New York and Chicago, Chicago and New Mexico,   New York to
New Orleans ((I forget by which route), New York to Virginia, Virginia to
Wisconsin and Wisconsin to New York. And something further west which I'm
not remembering at the moment, and I've taken long train rides along the
West Coast, which involved a bus connection which was run by Amtrack.  Of
course it takes longer but train stations are usually easier to get to than
airports,  you get a comfortable ride, and you kind of get an experience out
of it, not just a ride. However, it's not so good if you have a tight time
frame as trains are often delayed.
