Dear Parents and Professionals,
 
In a grass roots effort to raise awareness of Cortical/Cerebral Visual 
Impairment (CVI), we (two parents) have taken a first step by designing a CVI 
Survey to gather information how CVI is diagnosed, who diagnosed it,  
what information you were or were not given, what issues there may or not be 
related to school and what services adults and students are receiving or not 
receiving.   We invite you to complete the survey at your earliest 
convenience but no later than February 28, 2009 when the data collection will 
close.  All responses will remain anonymous and only the final results of 
the total survey will be released.
 
Please feel free to pass this link along to anyone you know who may have an 
interest in completing the survey.  One survey may be completed for each 
child with CVI that you have or that is on your caseload.
 
Should you have any questions/comments you may direct them to CVIAwareness at aol.com.
