Well you have an Index embosser. You should be able to go to the Index web site and find answers to your questions.
But unfortunately you cannot. The best, actually the only reasonable answer, to your question is to buy Duxbury braille translator and follow their instructions. The good Duxbury folks on this list can give more details.
If you had a ViewPlus Delta instead of the Index, you could get the answer from ViewPlus. Actually you don't really have to even ask that question. Just make your content in the way most sighted people do - with MS Word using the MathType math editor plug-in, translate with the TSS software that is bundled with every Viewplus embosser, and print it. After selecting UEB as your math option of course.

How best to create PDF, HTML, or other documents with mathematical content that 

(1) are accessible using screen readers such as JAWS and NVDA

(2) can also be translated into UEB mathematics braille so as to be embossed using the Index Braille's Everest-D V4 or V5 braille embossers?
And, how best to create XML files with mathematical content with the above two properties?


Please describe the full step-by-step procedure in detail for a blind and visually impaired person.
