What format is the fomr it?
 And yes, just learn the form, and ask through the conversation interview.
Try to ask questions which are open ended and can get more than just one
answer at once.

Hello, Quinto's suggestion is one which allot of us find out is the most
efficient, sighted or not.
 Read the form enough, and you can answer these questions through
conversation, provided they are not asking for specifics like SS and Address
of course...
