Hi,

I recently used surveymonkey.com to evaluate my college course.
the service allows you to create a survey comprising of 10 questions (free) or unlimited questions (subscription).
Service is completely accessible and you can embed the survey on your blog. share on social networks, email, or web link.
There is another service called poledaddy, but I haven't used this personally.
