4. how long have you had him/her?  5 and a half years.
5. what do you feed him/her?  Iams Lamb & Rice
6. if program trained did you train any extra tasks or commands if so what?
Mostly to find objects such as trash cans. 
8. how do you obtain dog food, store, order, etc.?  I have ordered from Pet
Food Direct and Petco.  Petco is more reasonable, but still, I find it a
good deal less expensive to have a friend take me to get it.  If my friends
are all busy, then I will order.

9. what state are you from?  Indiana
