Hi Sheila,
Avco stands for American college of veterinary ophthalmologists.  They 
do this as a service to all service animals and working dogs.  Once you 
register on their website, then you find the closest participating 
ophthalmologist nearest you.  You make an appointment with the 
ophthalmologist based on the time they have available.  At the time of 
your appointment, you bring in your printed out a registration form for 
your guide dog.
I hope that helps explain things about the free eye exam given every year.
