Have you tried a micro prong collar?  Many people with physical disabilities use them because it takes a very little effort from the handler to get the dog's attention. 
I like the smaller prongs better because they are less noticed than the larger ones.  You can also buy rubber tips for them if you wish.  You may purchase this kind of collar  from a petstore or online.  It's a training tool and should not be kept on the dog except for when the dog is working or you are Training.
