Good Day
Just saw your post, you would need the following documents when traveling to the Republic of Ireland and the United Kingdom. that your dog’s vaccines including Flea tick and heart worm is done at least 48hours before traveling to the Republic of Ireland, also that her rabies shot is up to date. You will be met by somebody from the department of agriculture in Dublin airport if that is where you are flying into and if not the other major airport in the Republic of Ireland is Shannon. They will be there waiting to meet you. Their office is beside customs in both airport, they will go through your documents and will scan your dog’s microchip.
Depending on how long both your self and your wife plan to stay in the Republic of Ireland you might not have to get your dog checked out by a vet before traveling to the United Kingdom as there is a special arrangement between both Countries when traveling with guide assistance dogs or service dogs but you would need to get your dog checked out if you are flying directly back to the United States from the United Kingdom, or if you are traveling back to the United States from the Republic of Ireland and it is less than 30days you will not need to put your guide through more Flea tick or heart worm treatments. The email address for the department of agriculture is livetrade at agriculture.gov.ie <mailto:livetrade at agriculture.gov.ie> this is the direct email for letting them know that you are traveling with a guide dog. I would suggest that you email them at least 72hours before departing, in subject field traveling with guide dog and in the body of email flight details that is flight number, and time of arrival you will arrive into terminal two in Dublin, you will receive a email from the department letting you know, that you will be met by one of their staff and your appointment, they are fairly good in appointment times as they know that people do not want to stay around airports when they arrive into a country.

I hope this information will help you on your journey to the Republic of Ireland and the United Kingdom next year. I live and work in the Republic of Ireland and I had no problems traveling to and from the US and the Republic of Ireland, which I will be doing again nest month.
regards
Gabriel
