Yes.

Yes, but I gaily tossed the envelope it came in before I realized it was
where my access code was.  How stupid was that! I bought it quite a while
ago, so I reckon it's too late to try and contact the maker and get a new
code. I got it to try and make this silly drive work.

Other answers:
I can't see the format anywhere, so I don't know what it is.

I had thought the drive worked, once, on the laptop, but it doesn't work
there now.
I bought the thing a couple years ago, and have been trying, off and on,
to get it to work since then, before giving up and letting it be a
paperweight again.
