Now doesn't Steven Hawking use something like that? I seem to remember it. Also, and I'll have to look up the contact information, there is at least one company that produces augmentative communication devices. I am not sure how advanced they are though.

Many years ago, the Boston Children's Hospital built a version of the DECtalk external synthesizer that worked with a keyboard to provide a "voice board" -- essentially what you're after. Obviously, that version of the DECtalk is no longer being made but that might be a place to start.

What you essentially want is what Steven Hawkings uses.
