Holly,

All states  have different procedures, but here's what I did:

I got a prescription from my doctor and brought to a wheelchair clinic. They 
evaluated me using one of their power chairs. After about six appointments 
where I practiced and showed them I could do it, the physical therapist oked 
it and helped me choose my chair. Of course, it would probably be good to 
have a mobility specialist there with you too.
