[Blindmath] MathType, word and reading existing documents

TeXToggle uses a few heuristics if there is an insertion point to guess
which expression to toogle (it looks near the insertion point for
something).  If you have a selection, there is no heuristic and it will
toggle all equations in the selection.  If you do a select-all, then it will
toggle the entire document (which takes a many seconds or more, depending on
the size of the document).
