[Blindmath] Capital letters in MathType equations?

I appreciate all the responses on the reading of capital letters. I need 
some more clarification though since I am not able to get it working. Also, 
the responses seem to suggest that the default mode should be to read 
capital letters with a pitch difference. But by default, I am not getting 
any pitch difference in equations, even though the same speech synthesizer 
is giving a very clear pitch difference in regular text.
Is there something I could be missing? Alternatively, is there a document 
with clear step by step instructions on where I can find the relevant math 
type settings? When I go to the Math type tab in word, I can't find any 
settings on capital letters.
