Hello Marry,

My name is Nikki Wunderlich, I live in Plymouth MN, and here you get it at 
the DMV You pay $18.00 or is it $18.50 for your first one and each one after 
that unless you have a special form which you can usually get from either 
your social worker or your doctor saying you can get it for the reduced 
price because it is just for identification not for driving and then it will 
only cost you $0.50 each time. I just got this done a few months ago. 
There's usually a DMV in most towns.
